APPOINTMENT SCHEDULING AND CANCELLATION POLICIES:
Sessions are typically scheduled to occur one time per week. A alternate schedule of therapy sessions may be recommended depending on the nature and severity of the client’s concerns. Consistent attendance greatly contributes to a successful outcome.
In order to cancel or reschedule an appointment, clients are expected to notify CrownTown Counseling at least 24 hours in advance of their appointment. If clients do not provide notice of cancellation at least 24 hours in advance, a fee of $50 will be incurred.
How we bill / fees:
50-minute individual sessions: $195
50-minute couples or family sessions: $245
80-minute couples or family sessions: $365
Payments are rendered at the time of each session (credit/debit cards, checks, and cash accepted). HSA/FSA debit cards are also accepted.
INSURANCE:
CrownTown Counseling is able to provide clients with a statement/receipt for paid services that clients can submit to their insurance company along with a request for reimbursement for an 'out-of-network' provider. It is recommended that clients contact their insurance company to find out their policy prior to receiving services.
CrownTown Counseling is a Lyra provider.
Good faith estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the No Surprises Act, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. Confidentiality:
The information disclosed by a client is generally confidential and will not be released to any third party without written authorization from the client, except where required or permitted by law. Exceptions to confidentiality, include, but are not limited to, reporting child, elder and dependent adult abuse, when a client makes a serious threat of violence towards a reasonably-identifiable victim, or when a client is dangerous to him/herself or the person or property of another.
If clients participate in couples or family therapy, confidential information about treatment will not be disclosed unless all person(s) who participated in the treatment provide their written authorization to release such information. However, it is important that clients know that CrownTown Counseling utilizes a “no-secrets” policy when conducting family or marital/couples therapy. This means that if clients participate in family, and/or marital/couples therapy, the therapist is permitted to use information obtained in an individual session that clients may have had, when working with other members of his or her family.
